Lactate in solid malignant tumors: potential basis of a metabolic classification in clinical oncology.

In all tumor entities investigated, high molar concentrations of lactate were correlated with a high incidence of distant metastasis already in an early stage of the disease. Low lactate tumors were associated with both a longer overall and disease free survival compared to high lactate lesions. Thus, lactate accumulation not only mirrors but also actively enhances the degree of tumor malignancy.

The focal areas showed a significant functional damage in lactate, pyruvate and succinate dehydrogenases and in the cytochrome oxidase activity when compared with the enzymic capacities of perifocal areas (normoxic as controls). We studied six infants with thiamine-responsive congenital lactic acidosis and normal pyruvate dehydrogenase complex activity in vitro, through clinical and biochemical analysis. Careful monitoring of lactate and pyruvate would be useful in determining thiamine dosage.
